    It is necessary to examine the root causes of occupational accidents experienced every day within the scope of the "2006/42 / AT Machinery Safety Directive". It should be aimed to spread the information to the base by determining the root causes and correcting the mistakes known as correct. Occupational accidents occur in the form of a series of events that show minor symptoms over time and that start when these symptoms are ignored, triggering each other.
    When looking at the causes of work accidents; basically, it can be divided into two parts: human errors and lack of information due to various factors.
    One of the most important causes of occupational accidents due to insufficient information is ignoring error masking that occurs during serial connection of safety equipment.
    The aim of this study is; In the scope of the technical report "Evaluation of Error Masking During Serial Connection of Interlocking Devices (ISO / TR 24119)", underlining the well-known mistakes about serial connection, conveying how the machine safety approach should be, stating how the error will cause problems. The solution suggestions required to eliminate this situation will be shared.
